Optical Character Recognition
The second option allows you to enable Optical Character Recognition (OCR), which is a machine learning-powered feature that allows Gmail to read text in JPG, PNG, GIF, or TIFF images for content compliance rule violations in attachments. Be aware that this will not work on images inside PDFs or Microsoft Word files, and that it's not 100% accurate, but still adds a nice layer of security:
